As we prepare for 2003, we know the year will be busy, exciting, challenging, fun, and filled with moments of high intensity and transformation. We will continue to work together to ensure that we remain essential in our customers' personal and professional lives and to leverage what the Internet does best in a unique way vs. other media and communications platforms.

We also will focus our efforts on the intangible aspects of building our organizational capabilities and providing the best customer/user experience, such as, providing better communications internally on our mission, vision, and values that can guide our strategy, and offering opportunities and initiatives that will build upon our unique culture so we will continue to be an "employer of choice".

We will look at improving how we work together to build our network of services and products. And to do all of this, we need your input as to what is most important for us in these efforts.

A key component in successfully evolving our culture is to have a clear understanding of where we are today. To that end, we are asking each employee (all full-time and part-time employees) to complete the attached Cultural Health Indicator?.

Employees are never shy about giving us their honest opinion, and that is exactly what we want to know, however, please answer the questions indicating our current state versus where we were in the past or aspire to be in the future. That will enable us to identify the key areas where there are gaps between our current culture and our desired culture.

The information will be tabulated and analyzed by Assessment Leaders, an outside company helping us with this project, so all individual responses will be completely anonymous. Summarized results of the survey will be shared when they are compiled in January.

This survey should take approximately 20 minutes to complete via the Internet. The survey is comprised of a series of objective statements and open-ended questions. The survey will be available online from December 5th until December 19th; therefore, please complete the survey online by December 19, 2002.

Please click on the Internet address below when you are ready to begin the survey. Once you log on, there will be additional instructions; therefore, please take the time to read them in detail before proceeding.

Should you need to leave the survey, you will be required to close your web browser and only sections that have been submitted will be saved and you can return to finish the survey later. Therefore, make sure you write your password down or print the page that contains your password.

This year is going to be pivotal in the evolution of XYZ -- busy, exciting, challenging, fun, somewhat stressful -- and full of change. In addition to the tangible changes we will undergo, like absorbing our new clients workloads and recruitment of many new staff, there are also many efforts focused on the intangible.

These include, most importantly, how we work together to build the very best home care consulting firm in, not only the Northeast but, all of America. We want to develop a culture that makes XYZ the most dynamic and exciting place to work in our area, and we need your continuing input in how to create that culture. A key component in successfully creating that future culture of XYZ, is to have a clear understanding of where we are today.

To that end, we are asking each employee to complete a Cultural Health Indicator?. This Index will be completed over the Internet and the information will be tabulated and analyzed by Assessment Leaders, an outside consulting firm helping us with this project.

All individual responses are completely anonymous. Please be as candid as possible in completing the survey, as no one at XYZ will have access to your individual response.

Summarized results of the survey will be shared with staff once they are compiled in February. This survey should take approximately 20 minutes to complete and must be done via the Internet; we will make Internet accessible PC's available to those employees who don't currently have access to the Internet.

The survey is comprised of a series of objective questions and three open-ended questions. Please complete the survey online by Friday February 1, 2001. Once you log on, there will be additional instructions; therefore, please take the time to read them in detail before proceeding.

RE: Creating an Acme Culture - the cultural survey

This year is going to be pivotal in the evolution of our company -- busy, exciting, challenging, somewhat stressful -- and full of change. In order to become a world-class organization we are embarking upon a significant initiative [ACME compliance]. This change process will have an impact on all of us. In addition to this tangible change, we must focus on the intangibles, including how we work together, how we make decisions, what our trust levels are, etc.

The decision to change how we do business must not be done in a vacuum; therefore, a critical component to creating an ACME culture is to look at our human system; in other words, our current organizational culture. We want to develop a culture that is truly focused and committed to ACME; and, we will need your input to achieve outstanding results.

A key component in successfully creating that future culture is to have a clear understanding of where we are today. To that end, we are asking you to complete the attached cultural survey. The information is being tabulated and analyzed by Assessment Leaders, an outside company helping us with this project. You will receive an email indicating the URL address to go to along with a company logon code and password. Additional instructions will be provided to you at the time of the actual survey.

You will log into a secure/private site and you will be asked to select your role at ACME from a number of drop downs which will include your division, line of business, site, function and whether you are an employee or a manager. There are no other demographics required; i.e. name, job title, etc., which means that this survey is totally anonymous! Also, it is very important that you recognize that ACME will not have access to the raw information that is collected. Summarized results of the survey will be shared with staff once they are compiled.

Our intent in conducting this survey is to gain a thorough understanding of our challenges and opportunities as we embark on this major initiative; therefore, your candid feedback is what we are interested in. We are not interested in who said what; but, rather what you are 'really' thinking.

The survey is comprised of a series of objective statements and three open-ended questions, which should take approximately 20 minutes to complete.

Finally, when responding to the statements, record what first comes to mind for you. Do not respond by how you wish things were; but respond to how things are today. Please do not over analyze the statements -- there are no trick statements. We are just looking for honest and genuine feedback. Remember the old saying "junk in means junk out."

Please complete the survey online by (insert date). If you are using the hard copy version, place it in the mail no later than (insert date) so it can be tabulated with other responses.

How are we doing? Everyone needs feedback and right now we need your feedback to understand how our company is performing in terms of overall organizational health and employee satisfaction. (fill in name), our CEO, has requested that each employee take a few minutes to complete this survey so that as an organization we can have a better overall picture of where our strengths and weaknesses are. Results of the survey and subsequent action steps will be shared with employees.

The survey will take you approximately 20-30 minutes to complete. If interrupted, you can save & return using the log-out/log-in feature. Assessment Leaders an outside company specializing in Organizational Culture will tabulate the survey results. Your results are submitted only by a web browser IP address; so individual responses can't possibly be identified and are completely anonymous, so please be honest and constructive! We want to hear from you!
